Apply online or download the application, complete the application, and then mail it to CTCOHS at:

Request for Official Transcripts

Fill out the transcript request form and send it to your previous high school(s) to have an official transcript and exit scores sent to CTCOHS. The transcript may also be faxed from your previous school to us with an official school cover sheet. We will need these to generate a diploma plan. CTCOHS Fax #: (254) 526-1819.

STUDENT HANDBOOK

The student handbook contains important information about CTC Online High School policies and is available for download. Students attending CTC Online High School are required to acknowledge download of the student handbook by signing and returning the acknowledgement form.

ACKNOWLEDGEMENT OF RECEIPT

Students attending CTC Online High School will need to acknowledge receipt of the Student Handbook. The student handbook contains important information about CTC Online High School policies and is available for download.

Proctor Application and PROCTOR REQUIREMENT

Certain tests, midterms, and finals will need to be taken in a proctored environment and it is the student’s responsibility to obtain an appropriate proctor. You should do this as soon as possible to prevent delays. Read the proctor requirements carefully, fill out the form, and send it back to CTCOHS. Remember to allow time for us to evaluate proctor suitability.

Request Transcripts from CTCOHS

CTCOHS will send an official transcript to another high school or college free of cost. You will need to fill out a separate form for each request. If you are a student who was enrolled with CTCOHS on or before January 1, 2014, please use this link to apply for a transcript. When complete, please send to the Graduation Inbox.

If you were admitted to CTCOHS after January 1, 2014, please visit this link to request a transcript. Use your WebAdvisor ID and temporary password (mmddyy) to access the documents. When completed, please send to the Graduation Inbox.

Diploma Application

Congratulations! You are almost finished. Fill out this form to receive your diploma. Your diploma and official transcript will be mailed to you.

 TRANSFER CREDIT FORM

Any student wanting to take some credits from CTCOHS and transfer the credit back to your high school              needs to download this form and have it signed by your parent/guardian and school counselor. Please fill out        an application and submit it along with the transfer credit form to begin taking your courses.
